PhenoPath Laboratories needs your NPI!
Recent HIPAA regulations require the use of standard unique identifiers for health care providers to facilitate the exchange of electronic health information. In accordance with these regulations, the Centers for Medicare & Medicaid Services (CMS) has established the National Plan and Provider Enumeration System (NPPES) to distribute and maintain National Provider Identifiers (NPIs). As of the compliance date, the NPI will be the only health care provider identifier that can be used for identification purposes in standard transactions by covered entities, and will replace any other provider identifiers currently in use, such as the UPIN. PhenoPath Laboratories is requesting that each institution and physician we partner with provide us with an NPI before the compliance dates. If we do not receive your NPI by the compliance date of May 23, 2007, we will be forced to bill you directly for Medicare and Medicaid patients, and possibly for third party payers who will require the use of your NPI number.
You can submit your NPI for an institution and/or up to 10 physicians using the form below. You can also download and mail a printer friendly version of the form. Alternatively, you can send an e-mail to npi@phenopath.com. Please be sure to include the physicians' full names, UPINs for cross reference, NPIs, phone numbers, and e-mail addresses, as well as contact information for the person we should communicate with to answer any questions we may have.
